Welcome to ADHDTestNow. Please read these Terms of Service (“Terms”) carefully before using this website or any associated services.

By accessing or using ADHDTestNow, you acknowledge that you have read,understood, and agree to be bound by these Terms.

1. Service Description
ADHDTestNow provides access to an objective, computer-based ADHD assessment intended to support clinical evaluation and informed decision- making. The service provides performance-based assessment data and related
information.

The service does not provide medical advice, diagnosis, or treatment.

2. No Medical Diagnosis or Treatment
ADHDTestNow does not independently diagnose ADHD or any other medical or psychiatric condition. Assessment results are intended to be used as an aid to a broader clinical evaluation conducted by a qualified healthcare professional.

Use of this service does not replace consultation with a licensed healthcare provider

3. No Provider-Patient Relationship
Use of ADHDTestNow does not establish a provider-patient relationship. Any such
relationship exists only if and when you separately engage a licensed healthcare
professional under applicable law.

4. Eligibility and Geographic Availability

ADHDTestNow services are currently available only to individuals physically located in:

• Texas

• Colorado

• Arizona

• Minnesota

By using this service, you represent that you are located in one of these states at
the time of use.

5. Fees and Payment

The standard fee for the ADHDTestNow assessment is $325, unless otherwise stated. Promotional discounts may be offered for limited periods and are subject to change without notice.

All fees are due prior to access to the assessment and are non-refundable except
as required by law.

6. FDA-Cleared Technology

The assessment utilizes technology that is FDA-cleared as an aid in the assessment of ADHD. FDA clearance does not indicate that the assessment independently diagnoses ADHD or determines treatment.

7. User Responsibilities

You agree to:

• Provide accurate and complete information

• Use the service only for lawful purposes

• Not misuse, reverse-engineer, or interfere with the assessment platform

8. SMS Communications

By opting to receive text messages, you consent to receive informational communications related to ADHDTestNow. Message frequency may vary. Message and data rates may apply. Text STOP to opt out. Text messaging does not provide medical advice and does not establish a
provider-patient relationship.

9. Limitation of Liability
To the maximum extent permitted by law, ADHDTestNow and its operators shall not be liable for any indirect, incidental, or consequential damages arising from use of the service.

10. Governing Law
These Terms are governed by the laws of the State of Delaware, without regard to conflict-of-law principles.

11. Ownership and Operation
ADHDTestNow is owned and operated by Diamond Global Healthcare, a Delaware corporation.

I understand that the ADHDTestNow assessment is a computer-based, objective assessment designed to measure attention, impulsivity, and activity during a structured task. The assessment is intended to support clinical evaluation, not to independently diagnose ADHD.

No Diagnosis or Medical Advice

I understand that:

• The assessment does not provide a medical diagnosis

• Results do not replace evaluation by a qualified healthcare professional

• Medical decisions should be made only by licensed providers
  

FDA-Cleared Technology Disclosure

I understand that the assessment utilizes FDA-cleared technology intended to
aid in ADHD assessment and monitoring, and that FDA clearance does not imply
guaranteed outcomes or diagnoses.

Voluntary ParticipationI

I understand that:

• Participation is voluntary

• I may choose not to proceed at any time prior to completion

• There is no obligation to pursue treatment or additional services

Data Use and Privacy

I understand that my assessment data will be handled using secure systems and
in accordance with applicable privacy standards. Results will not be shared
without authorization, except as required by law.
